How Custom Oral Appliances Work
Mandibular advancement devices (MADs) are oral appliances designed to treat obstructive sleep apnea (OSA) and snoring. They function by repositioning the lower jaw (mandible) forward during sleep. They must be custom made for each user and a trained dentist should oversee treatment. This forward shift tightens the soft tissues and muscles of the upper airway, particularly the tongue and soft palate. As a result, the airway is less likely to collapse or vibrate, which reduces both apnea events and snoring.
MADs are clinically validated for patients with mild to moderate obstructive sleep apnea (AHI between 5 and 30) and individuals with upper airway resistance (UARS). CPAP and other solutions like Inspire are often recommended for more severe cases.
✔️ Randomized controlled trials have shown that MADs significantly reduce AHI (Apnea-Hypopnea Index), especially in patients with lower BMI and who have positional apnea.
✔️ Meta-analyses (e.g., Ramar et al., 2015; Phillips et al., 2013) have found that while CPAP is more effective in reducing AHI, MADs are often preferred due to greater comfort and adherence.
✔️ The American Academy of Sleep Medicine (AASM) and the American Academy of Dental Sleep Medicine (AADSM) both endorse MADs as first-line therapy for: 1) Mild OSA 2) Moderate OSA when CPAP is not tolerated or refused
